Maintain daily contact with key hospital departments, physicians and their staff, discharge planners, social workers, and community representatives. Provide educational material concerning home care guidelines, Company services, and how Haven Home Health can assist their patients. <br> • Initiate and maintain relationships with senior associates, community representatives, and area health care facilities. Discuss and share new developments in home care. <br> • Maintain ongoing communication with employees of Haven Home Health to ensure the needs of the community and healthcare staff are being met. <br> • Assist in the development of new services, processes, or procedures to meet the needs or improve upon patient care.
